I am registering for the full conference – both Thursday evening (10/18) and Friday (10/19)
I am ONLY registering for Friday (10/19)
I plan to make a reservation for Thursday night (10/18) at the Holiday Inn Select Executive Center
I will NOT make a reservation at the Holiday Inn Select Executive Center

$150 – General Registration (before September 1)
$75 – MCPSA Member Registration (before September 1)
$200 – General Registration (after September 1)
$100 – MCPSA Member Registration (after September 1)

Accommodation and Dietary Needs

Please provide information regarding any accommodation or dietary needs in the box below (i.e. request vegetarian meals). We are glad to assist however we can.
